Registered Nurse (RN)
- Providing comprehensive nursing care to patients in there home.
- Assessing their health needs, and developing personalized care plans.
- You will administer medications, monitor vital signs, and educate patients and their families on managing health conditions.
- Additionally, you will collaborate with other healthcare professionals to ensure continuity of care.
- Maintain accurate patient records.
- Your role also involves advocating for patients and supporting their emotional and physical well-being in a home environment.
CCP SUPERVISOR

Provide basic nursing care, including wound care, medication administration, and vital sign monitoring.

Conduct health assessments and report changes to supervising healthcare professionals.

Educate patients and families on managing chronic conditions and care routines.

Implement care plans as directed by RNs or physicians.

Document patient care and progress accurately.

Coordinate with other healthcare professionals for comprehensive care.

Evaluate home safety and recommend improvements.

Offer emotional support to patients and families.

Follow infection control protocols.

Respond to patient emergencies and provide first aid if needed.
